Transcripts and Enrollment
Instructions for Ordering Transcripts
Official Transcripts for Currently Enrolled Students:
Current students may request a copy of their official transcript by completing the request form in the main office. Transcripts are free for currently enrolled students.
Official Transcripts for Previous Students and Graduates:
Requests must be made in writing (via the form in the main office, email, fax, or letter) and include the following information:
- Full name at the time of attendance
- Date of birth
- Photo ID
- Address or fax number where transcripts will be sent
- $2 for each transcript. Cash or check accepted; please makes checks payable to South Medford High School. This $2 fee can be paid in person through the bookkeeper or mailed to the schools address listed below. At this time SMHS is unable to accept payments online.
