Students will not need to turn in physical forms. You can upload the following forms during the online registration process. If you would like to email or turn in physical forms to your neighborhood school, there will be an option for that as you enroll online.

New Students

·         Birth Certificate or Court Affidavit with student’s legal name & DOB (first time enrolling in school only)

·         Immunizations

·         Proof of Residency 

Re-enrolling or returning students may need to provide:

·         Proof of Residency

·         Immunization updates
